BACKGROUND: Interleukin-6 (IL-6) is a multifunctional cytokine which regulates immune responses and host defence mechanisms. IL-6 has been found to be increased in certain inflammatory conditions of the kidney, in which tubular epithelial cells play a pivotal role. Human renal tubular cells express IL-6. Until now no data about the effect of the immunosuppressant drug mycophenolic acid (MPA) on IL-6 expression were available. METHODS: Proximal and distal tubular epithelial cells (PTC/DTC) have been isolated immunomagnetically. Confluent monolayers were stimulated with interleukin-1beta (IL-1beta; 25 U/ml), IL-1beta+ MPA (0.25-50 micro M) or MPA alone for 48 h. Release of IL-6 protein into the supernatant was evaluated with an enzyme immunoassay, IL-6 mRNA expression was evaluated using the Quantikine mRNA kit. RESULTS: After IL-1beta stimulation, a highly significant 2.6- (PTC) and 3.8-fold (DTC) upregulation of IL-6 expression was detectable. IL-6 mRNA was upregulated by IL-1beta [1.57- (PTC) and 2.03-fold (DTC)]. MPA inhibited this cytokine-induced IL-6 expression in a dose-dependent manner. Incubation with the lowest MPA concentration had no effect on the stimulated upregulation, whereas all higher doses significantly decreased IL-6 expression. Dexamethasone significantly inhibited the cytokine-induced IL-6 protein release in PTC, but not in DTC. CONCLUSIONS: In this study we demonstrated for the first time an inhibitory effect of MPA on the stimulated IL-6 expression of renal tubular epithelial cells. In contrast to older data, which showed a synergistic upregulation of the expression of a CC-chemokine by a combination of cytokines and MPA, in the present study we could demonstrate an immunosuppressive effect of MPA on the expression of an important cytokine.  相似文献

After removal of intraductal stones, a 10‐Fr or 7‐Fr pancreatic stent was placed in 16 patients with upstream ductal dilation proximal to a stricture of the main pancreatic duct. Stents were removed after a mean duration of 52.5 days. Nine patients underwent repeated stenting. About one year after removal of the initial stent, when the remaining upstream ductal dilation was found on follow‐up pancreatograms, the next stent was replaced. Repeated stenting improved outflow of pancreatic juice more effectively than one‐time stenting. Correlation between long‐term pain relief without recurrence of intraductal stones and reduction of duct diameter was also shown. Stent occlusion was observed in 14 of 30 stents. Stent occlusion was frequently associated with recurrence of pancreatitis and intraductal stones, and was also associated with morphologic changes in the pancreatic ductal system. Although there were no significant differences between stent patency of the initial stents and that of the next stents, stent patency of 10‐Fr stents was superior to that of 7‐Fr stents. 10‐Fr stents should be removed within 8 weeks and 7‐Fr stents should be removed within 4 weeks for the prevention of stent occlusion. Repeated stenting with short‐term stenting is therefore considered a safe and effective protocol of endoscopic pancreatic stenting.  相似文献

目的:评价GAMMA钉治疗股骨转子间骨折的效果。方法:对32例股骨转子间骨折采用GAMMA钉治疗。结果:经6~18月随访,所有病例均获骨性愈合,下肢功能及关节活动良好。其中1例为粉碎性骨折内固定后出现髋内翻,按X线评分标准90%病人满意。结论:GAMMA钉治疗股骨转子间骨折效果满意。  相似文献

【目的】探讨股骨干骨折扩髓与不扩髓髓内钉固定对凝血机能和肺换气功能的影响。【方法】 38例股骨干骨折分别采用扩髓 (RFN)和不扩髓 (UFN)髓内钉固定 ,在手术开始前至术后 4 8h内 7个不同时间段监测血小板、纤维蛋白原、凝血酶原时间 (PT)和部分凝血活酶激活时间 (APTT)的变化 ,并动脉血气分析 ,计算肺泡 动脉血氧分压递差 [P(A aDO2 ) ]。【结果】RFN组 ,扩髓后血小板开始减少 ,P(A aDO2 )升高 (P<0 0 5 ) ;髓钉插入后 30min纤维蛋白原减少 ,PT、APTT延长 (P <0 0 5 ) ;UFN组表现出同样的趋势 ;两组中各项指标均在 2 4~ 4 8h内恢复正常 ;动脉血气分析各时间段和组间差异无显著性 (P >0 0 5 )。【结论】扩大髓腔的髓内钉固定 ,和不扩髓相比 ,并不影响肺换气功能 ,对凝血机能的影响是一个自限过程 ,单纯股骨干骨折 ,可耐受扩髓固定手术。  相似文献

Bone mineral density (BMD, g/cm2) was measured using dual-photon absorptiometry (DPA) in selected areas of the proximal tibia following uncemented PCA knee prosthesis. In nine patients with 14 alloplastic operations, measurements were taken at 3-6-month intervals for the first 3 1/2 years after operation. There was a significant increase in BMD of about 15% during the first 6 months after operation. The following year it remained increased, although not significantly, compared with the initial values, then gradually diminished. Increased bone density after arthroplasty may be explained mainly by stimulation of bone formation from weight bearing due to improved walking ability. Stress shielding of the proximal part of the supporting tibial bone did not seem to occur.  相似文献

目的 通过体模检测和病例检查的对照研究,比较计算机X射线摄影(computed radiography,CR)新旧两种不同成像板(IP)对影像质量和病人辐射的影响,探讨新旧IP的使用原则。方法 对新旧IP进行常规暗噪声、均匀性和一致性、擦除完全性检测;根据阈值对比度-细节检测能力(TCDD)技术,75 kV时新旧IP对体模TO.16进行摄影,请3位观察者采用双盲法对所得影像进行观察评分,计算阈值检测指数HT(A);结合临床用相同曝光条件,不同的IP分别对膝关节、胸部、腰椎进行投照,请3位放射专家进行双盲阅片,评价影像质量。结果 新旧IP在物理指标上基本相同,在相同曝光条件下,新IP的检测能力高于旧IP,当旧IP提高20%的辐射剂量后,旧IP检测能力接近新IP。结论 随着IP使用次数的增加,需逐步提高曝光剂量才能保证影像质量;若保持原有剂量不变,IP的检测能力就会下降,将影响疾病的诊断。  相似文献

[目的]观察骨形成蛋白(bone morphogenetic protein,BMP)、碱性成纤维细胞生长因子(basic fibro-blast growth factor,bFGF)与异体脱抗原松质骨(allogeneic antigen-extracted cancellous bone,AACB)复合后修复股骨头坏死(femoral head necrosis,FHN)病灶清除区缺损,评价bFGF对FHN的再血管化作用及其与钙含量的关系。[方法]取18只杂种犬共36侧股骨头,建立液氮冷冻诱导性犬股骨头缺损坏死模型,随机分为A、B、C3组,每组12侧。A组为空白对照,B组植入AACB/BMP,C组植入AACB/BMP/bFGF。每个股骨头内植入AACB约0.33g,BMP约12.5mg,bFGF约2000U。术后3、6和12周分批处死动物,每组每次处死2只。行组织学观察,免疫组织化学染色,进行血管计数和血管面积图像分析、钙含量测定、并分析再血管化作用与钙含量间的关系。[结果]组织学观察,12周时A组以纤维结缔组织为主,B组多量新生骨组织形成,密度不均,C组股骨头骨缺损完全修复。血管计数和血管面积,C组术后3周移植物孔隙内大量血管增生,6和12周时血管数量和血管面积进一步增加,各时间点都大于A、B组,具有统计学意义(P<0.05)。钙含量,C组术后3周钙含量小于B组,12周时大于B组(P<0.05)。再血管化作用与钙含量的关系,C组与B组比较,术后3周时再血管化与钙含量呈负相关,12周时呈正相关。[结论]AACB是生长因子的良好吸附载体,适宜新生血管长入。吸附有bFGF及BMP的AACB具有较强的再血管化能力,bFGF促进植骨材料吸收,加速新骨形成。这一疗法有望成为FHN治疗的一种手段。  相似文献

A 13-year-old boy who had hemophilia A was reported with pain in the left thigh and hip on walking. He had no history of trauma. Severe hemophilia A is diagnosed with a Factor VIII level of <1 iu/dl. The presumptive diagnosis was that of a spontaneous bleed into the hip joint. Factor VIII mutational analysis revealed a C to G substitution at nucleotide 6683 which results in a cystine change at codon 2194. However, the symptoms persisted and an X-ray demonstrated the presence of an acute on chronic slip of the upper femoral epiphysis. The patient was transferred to the center treating his hemophilia where the hip was pinned in situ under cover with Factor VIII. This case demonstrates the need to be aware of a possible traumatic diagnosis of hip pain in a hemophiliac child with a longstanding history of spontaneous bleeding into joints.  相似文献

目的探讨经皮微创锁定加压钢板内固定术的临床应用及其疗效。方法应用间接复位技术,通过建立骨折两端皮下隧道,采用锁定加压钢板内固定治疗胫腓骨干骨折15例,锁骨骨折12例,肱骨骨折4例。结果所有患随访4~18个月,平均10个月。骨折愈合时间:胫骨10~20周(平均12周),1例骨折延迟愈合(20周),无骨不愈合;锁骨6~10周(平均8周),肱骨8~13周(平均11周),均无骨折延迟愈合及不愈合。所有病例无感染及内固定失败等并发症。结论经皮微创锁定加压钢板内固定符合生物学固定(BO)原则,有利于骨折的愈合及软组织的修复。  相似文献
